The numbers are irreversible, and they are damning.

A new study published this offseason finds that NFL players are nearly four times more likely to die of neurodegenerative disease than the general male population. That is a 4.0x multiplier -- not 1.4x, not 2.5x. Nearly four times. And that single figure reframes every padded practice, every protocol tweak, and every commissioner's statement about how much safer the game has become.

This is not a study about helmet technology in 2026. It is about a population -- roughly 20,000 men who have played in the NFL -- whose mortality data tells a story that no public-relations campaign can massage.

Here is what the data says, why it matters, and what it doesn't say.

The Multiplier That Cannot Be Explained Away

The nearly fourfold risk is the headline, but the real weight is in the denominator. Neurodegenerative diseases -- including ALS, Alzheimer's, Parkinson's, and chronic traumatic encephalopathy (CTE) -- remain relatively rare causes of death in the general population. Multiply that baseline by four, and you get a cause of death that is no longer rare in the former-player population.

This is not a new hypothesis. The Boston University CTE Center has been documenting brain pathology in deceased football players for over a decade. The NFL's own 2020-21 data showed a 2.6x higher rate of death from neurodegenerative causes among retirees. The new study pushes that figure higher, with a larger sample and tighter controls.

Why the discrepancy? Earlier studies often relied on smaller cohorts or self-reported data. This one used death certificates -- a more complete but also more lagging indicator. Brains damaged by repetitive head trauma do not stop deteriorating when the player retires. The risk accumulates. The current fourfold figure likely captures the real long-term toll more accurately than league-funded studies that point to lower rates.

What This Does -- and Does Not -- Mean for the Current Game

The average subject in this study played mostly in an era when concussion protocols were nonexistent, when returning to the field after getting hit in the head was considered a badge of honor, and when defensive linemen were taught to use their heads as battering rams. That era is behind us. But the question the league and the players' union need to answer is: by how much has the risk been reduced?

The NFL has fundamentally changed rules on kickoffs, defenseless-player hits, and lowering the helmet as a weapon. The data from 2023 through 2026 on diagnosed concussions shows a modest downward trend. But the brain does not respond solely to diagnosed concussions. What matters is the number of sub-concussive hits -- routine collisions on every running play, screen pass, inside-zone block -- that transfer force to the brain without producing diagnosable concussions.

That number has not significantly dropped. Offensive linemen still engage in physical contact on nearly every offensive snap, linebackers still fill gaps aggressively, and the physics of collisions remain unchanged. A collision between two large athletes at high speed transfers the same force regardless of new rules.

The Rating Gap the Data Exposes

From an analytical standpoint, the league's injury surveillance has underestimated the true risk. The NFL's system tracks diagnosed concussions but cannot track the cumulative sub-concussive hits that are believed to cause long-term damage. So, while diagnosed concussions have decreased by about 10% over the last four seasons, that reduction does not directly translate into an equal reduction in the risk of neurodegenerative diseases.

Testing that correlation would require following players over a generation -- 15 to 20 years of reduced head-impact exposure -- before any clear decreases in mortality rates can be confirmed. Meanwhile, the current cohort of retirees continues to age into the risk window, reinforcing the importance of these findings.

What the League Should Actually Do With This

A nearly fourfold increase in neurodegenerative disease mortality is not a public relations problem; it is a structural one that demands action.

The NFL Players Association has advocated for independent spotters, video review systems, and mandatory baseline neurological testing. While these efforts help identify symptoms more accurately, they treat the symptoms, not the cause. The cause is the sheer volume of high-force collisions per player per season.

To truly address the problem, the league needs to consider more disruptive solutions: reducing the number of full-contact practices per season, shifting youth and high school football toward flag formats, and fundamentally changing the way the game is played at its core. These measures are controversial and not quickly implemented, but the longer they are delayed, the more players accumulate exposure that will result in irreparable brain damage.

My Verdict

This study represents the most comprehensive longitudinal evidence publicly available on the neurodegenerative risk of playing in the NFL. It is not an attack on the league but a clear measurement of risk.

A fourfold increase is neither a statistical artifact nor a rounding error. It is a stark number that will influence future collective bargaining agreements, ongoing litigation, and parents' decisions about allowing their children to play tackle football.

The league's smartest response is not to question the methodology or denigrate the findings but to invest aggressively in innovations beyond helmet technology. This includes rule changes to reduce dangerous collisions and funding for longitudinal tracking of brain health across player generations.

Because as of today, the fourfold risk is the single most important number in the NFL -- and it has nothing to do with who wins the Lombardi Trophy.
